Download the signature page (Microsoft Word document), fill it out, print it, sign it, and submit it by campus mail to the chair of the CHSR, Dr. Stephanie Furrer. Be sure that the project title, name of the Principal Investigator, and date are the same as in the project proposal form, since these items are used to link the signature page to the proposal.
