Standard Project Proposal Form
Lead Time for Proposal Review
To ensure adequate time for consideration of proposals and verification of any modifications that may be required, please submit proposals at least five business days before data collection is scheduled to begin (or interviews are to be arranged) for projects that clearly qualify as exempt or expedited, and at least ten business days in advance for projects that may be subject to full review.
Directions
Download the proposal form, fill it out, and submit it by email to the chair of the CHSR using the email address CHSRChair@hastings.edu. Be sure to download, fill out, print, and sign the signature page as well and send it by campus mail to the chair of the CHSR.
